SouthLight’s Community Support Team offers an intensive, community based team approach to individuals with complex treatment needs who have a primary Mental Health and/or Substance Use diagnosis. CST increases an individual’s ability to live successfully in the community.
CST assists individuals with:
- Skill development of interpersonal, community, coping and independent living skills
- Symptom management
- Psychoeducation
- Crisis intervention
- Community integration
- Coordination of care
What to Expect
- Up to 6 months of In-Home/Community services
- Face-to-face contacts each week
- Access to first responder crisis response and interventions
- Structured Team approach to therapeutic services
- Linkage to community resources
The Team
Team members combine their unique skills to collaboratively support individuals in the ongoing
development, implementation and revision of their Person-Centered Plans. Each team has four members: Team Lead, Substance Use Professional, Qualified Professional or Associate Professional, and Certified Peer Support Specialist.

Who Qualifies?
Individuals with Medicaid and state funding that are 18 years of age or older with a primary mental health and/or substance use disorder. Individuals must meet eligibility criteria as outlined by the Community Support Team service definition.
